Remember: Before beginning the next two steps, you must have completed Step 1: Complete and submit the Southern Illinois University Edwardsville School of Pharmacy Application for Admission. If you have not done so, click HERE.
Step 2.
Complete and submit the GPA Calculation Forms. Using your own copies of your post-secondary transcripts, complete the three Grade Point Average Calculation Forms. If you have questions about how to complete these forms, call the SIUE School of Pharmacy Office of Student Affairs at 618-650-5150.
The application deadline for receipt of all elements of your application, including the Online Application, GPA Worksheets, Letters of Recommendation, PCAT Score, and Transcripts is December 1, 2009. You can check which elements of your application have been received by visiting the 'Check My Application Status' on CougarNet. Allow 3-5 business days for the information on this site to update.
