<p class="bodytitle" align="center"><span class="bodytext"><strong>2012 Application Form</strong></span></p>
<p class="bodytext" align="center">Ina Peabody Sledge, a native of East St. Louis, Illinois, served with distinction as Education Librarian at Lovejoy Library, from 1983 until her death in February 1998. During her 30 years of service to SIUE, she worked in a number of positions including head of the East St. Louis Center Library. Ina Sledge’s caring, giving attitude motivated and encouraged students to pursue their educational goals. She recognized and valued strong character and moral fortitude.</p>
<p class="bodytext" align="center">The Ina Peabody Sledge Award is given in the spring. There is a $500 stipend with the Sledge Award. SIUE undergraduate students, in good standing, with a cumulative grade-point average of 2.5 or above are eligible to apply for the Award. Preference is given to residents of Alorton, Brooklyn, Centreville, East St. Louis, Venice, and Washington Park. Applicants will be selected on the following criteria that embody the positive ideology of Ina Sledge:</p>

<p class="bodytext" align="left"><strong class="bodytitle"><em>Friends of Lovejoy Library Honors Award ($500)</em></strong><br /> The Honors Award is given for scholarship, based on grade-point average, during enrollment at SIUE. Only undergraduate students are eligible for this award. Please include the following when applying:<br /> 1. Transcript for all attendance at SIUE (available on CougarNet)<br /> 2. Recommendation from supervisor.</p>
<p class="bodytext" align="left"><strong class="bodytitle"><em>Carol Ammon - Friends of Lovejoy Library Student Development Award ($500)</em></strong><br />The Student Development Award is given to a student who, on the basis of evaluation by supervisor and written letter, has shown a progressive development of job skills and work performance while maintaining superior grades. Graduate and undergraduate students are eligible. Please include the following with this application:<br /> 1. Transcript for all attendance at SIUE (available on CourgarNet)<br /> 2. Recommendation from supervisor<br /> 3. Personal letter addressing the influence of the work experience at Library and Information Services.</p>
<p class="bodytext" align="left"><strong class="bodytitle"><em>Graduate Library School Award ($500)</em></strong><br />The Friends of Lovejoy Library provide an award to an SIUE student or SIUE alumnus who is planning to attend a graduate library science program after graduation. The award stipulations are as follows:<br /> 1. The awardee must either be a SIUE graduate or assured of graduation no later than the end of summer session the year the award is given.<br /> 2. The awardee must be accepted for admission to a Master's Program in Library Science accredited by the American Library Association in the fall semester following graduation.<br /> 3. Applicants must submit a letter of interest relating to their future plans in librarianship, a transcript, and a letter of acceptance from an ALA accredited library program.</p>
<p align="left"><strong>Please submit required materials to the Friends of Lovejoy Library by February 24, 2012</strong>. Winners will be notified by mail. Award certificates will be presented at the Friends of Lovejoy Library Annual Meeting and Dinner on May 11. Award checks will be deposited directly into the winner’s student account at the Bursar’s office in April or early May.</p>
