Each of the following files is required to apply for admission to the School of Pharmacy. Before proceeding with your next selection, please download each of the following files to submit with your application:

Pharmacy Online Application Instructions
School of Pharmacy Recommendation Form (Print two copies.)
School of Pharmacy Accreditation Disclosure Statement

Please refer to the Pharmacy Online Instructions for direction in completing the GPA Calculation Forms. The Grade Point Average Worksheets (Cumulative, Pre-Pharmacy, Pre-Pharmacy Science and Math; click to download) should be completed online and then printed for submission.

After you have downloaded the above-listed files, please proceed by selecting one of the following:

I am a U.S. citizen or a permanent resident of the U.S.; I wish to apply to the SIUE Pharmacy program, and am not currently attending SIUE, and I have met all the pre-pharmacy requirements.
I am a U.S. citizen or a permanent resident of the U.S. now attending SIUE; I wish to apply to the SIUE Pharmacy program, and I have met all the pre-pharmacy requirements.
I am not a U.S. citizen; I wish to apply to SIUE for the first time to the Pharmacy program, and I have met all the pre-pharmacy requirements.
I am not a U.S. citizen; I am now attending SIUE and wish to apply to the Pharmacy program, and I have met all the pre-pharmacy requirements.
