Each of the following files is required to apply for admission to the School of Pharmacy. Before proceeding with your next selection, please download each of the following files to submit with your application:
Pharmacy Online Application Instructions
School of Pharmacy Recommendation Form (Print two copies.)
School of Pharmacy Accreditation Disclosure Statement
Please refer to the Pharmacy Online Instructions for direction in completing the GPA Calculation Forms. The Grade Point Average Worksheets (Cumulative, Pre-Pharmacy, Pre-Pharmacy Science and Math; click to download) should be completed online and then printed for submission.
